Cancellations & Rescheduling

Please remember to cancel or reschedule 24 hours in advance. Cancellations and rescheduled sessions will be subject to the full service fee if NOT RECEIVED AT LEAST 24 HOURS IN ADVANCE. This is necessary because a time commitment is made for you and is held exclusively for you. If you are late for a session, you may lose some of that session time.

Confidentiality

Mending Mind Body & Relationships, LLC and Rhoda Mauclair-St Louis take clients confidentiality seriously. All clients service and treatment information are protected by the NASW code of ethics and under the Health Insurance Portability and Accountability Act of 1996 (HIPAA). As a mandated reporter in cases involving safety concerns, child or elder abuse and/or neglect, a report will be made with the local authorities.

Good Faith Estimate / No Surprises Act

As per federal mandates, a “Good Faith Estimate” (GFE) must be provided when uninsured or self-pay client/patients request an estimate, discuss costs, or schedule a procedure/treatment. Prior to scheduling service, clients are entitled and will be provided a GFE form.
